Debbie A. Cihak, MD Internal Medicine
Play My VideoDr. Debbie Cihak received her Doctorate of Medicine degree from University of Illinois College of Medicine during which time she was inducted into Alpha Omega Alpha National Medical Honor Society. She completed her Internal Medicine residency training at Maricopa Medical Center in Phoenix, Arizona and Cottage Hospital in Santa Barbara, California where she was offered the Chief Resident position. She has been board certified in Internal Medicine (ABIM) since 1990. Dr. Cihak has been practicing in the Galena area since 1991 working both primary care and most recently focusing on hospital medicine where she helped establish the first Hospitalist program in Dubuque, Iowa. She joins Midwest Health Clinic to return to the office setting specializing in diseases of adults both for preventative care and for management of complex medical problems with a special interest in hospital medicine and coordination of multi-specialty care.

Michael Wells, MD Family Medicine
Dr. Michael Wells is Board-Certified in Family Practice. He is a native of Freeport, IL and now lives in Galena with his wife and three children. Dr. Wells obtained his medical degree in 1998 from the University of Illinois School of Medicine. He was in the first full class of the Rural Medical Education program (RMED) at the Rockford campus. In 2001, he finished his residency with the University of Colorado at the Swedish Medical Center near Denver. After residency, Dr. Wells worked at Conifer Medical Center, an eight physician practice about an hour from Denver. After their first child was born, Dr. Wells and his wife decided to move back to Illinois to be closer to their families. In 2007, Dr. Wells accepted a position with the newly established Midwest Health Clinic, and was appointed to the faculty of the University of Illinois College of Medicine at Rockford as a Clinical Assistant Professor of Family Medicine.
Matt Gullone, MD Family Medicine
Dr. Matt Gullone received his Doctorate of Medicine from the University of Illinois College of Medicine. He completed his residency and was chief resident, in South Bend, IN with Memorial Family Medicine. He received outstanding marks for Essentials of Patient Care and Internal Medicine. Dr. Gullone is the recipient of numerous awards, including Gary Button Rural Primary Care Award and the Gold Humanism Honor Society in Medicine. Dr. Gullone joins the team of providers at Midwest Health Clinic with a special interest in men’s health, sports medicine, and comprehensive family health care. He has special training in Sports Medicine and is ACLS Certified. His research included determining the most common injuries related to distance running events and developing interventions to help them, as well as assessing the causes related to the onset of childhood obesity.
Beth Gullone, MD Family Medicine
Dr. Beth Gullone received her Doctorate of Medicine from the University of Illinois College of Medicine and completed her residency in South Bend, IN with Memorial Family Medicine. She is also summa cum laude by way of Loras College in Dubuque, IA where she received her B.A. in Spanish with a minor in Chemistry. Dr. Gullone is the recipient of such prestigious awards as the William H. Lowell Memorial Award of Primary Care, the Gold Humanism Honor Society and she received top honors for Outstanding performance in Obstetrics/Gynecology and Pediatrics to name a few. Dr. Gullone is certified in PALS(pediatric advanced life support) and ACLS(advanced cardiac life support) and is fluent in Spanish (Se habla español). Her practice with Midwest Health Clinic includes family medicine with a strong interest in women’s and children’s health.
Peg Dittmar, FNP-BC Family Nurse Pracitioner
Margaret (Peg) Dittmar has been a resident of Jo Daviess County for most of her life. She and her husband enjoy the country life and are involved with family which includes children and grandchildren. Peg received her Nursing education from Highland Community College (RN), Rockford College (BSN) and Northern Illinois University (MSN) which included specialty training for Nurse Practitioners. She is currently a nationally Board Certified Advanced Practice Nurse with specialty training in Family Practice. Peg is a member of the Nurse Advisory Board for the nursing programs of Highland Community College and the Saint Anthony School of Nursing.
Peg’s experience in patient care included certifications in Pre-hospital (EMS-Paramedic), Emergency and Trauma (CEN, FNTC), Intensive Care (CCRN), as well as a Fellowship in Cardiovascular Nursing. She holds certifications in Pediatric Advanced Life Support, Basic Life Support and Advanced Cardiac Life Support. A recent addition in certification is a SANE (Sexual Assault Nurse Examiner).
Peg enjoys being involved on a community level. She has been a Directory on the Jo Daviess Country Health Department Board, and is currently on the county Cancer Coalition, the Sexual Assault Response Team (SART) and the Local Emergency Planning Committee.
